Acceptable Visa Classes
1. A-1 through A-3
- a. An “A” visa holder is eligible for a permit/license/ID card only if the customer presents a letter from the Office of Foreign Missions (OFM) stating that he/she is not eligible for a United States State Department license and meets all identification requirements.
2. DV (new)
3. E-1 through E-2
4. F-1 through F-2
- a. The visa holder must present an I-20 certificate of eligibility.
- The I-20 form(s) must meet the following requirements:
- i. The I-20 is stamped by the United States Citizenship and Immigration Services (or formerly Immigration and Naturalization Service)
- ii. The I-20 states that the customer is currently enrolled in a Connecticut, Maine, Massachusetts, New Hampshire, New York, Rhode Island, or Vermont school.
- If the customer has multiple I-20s, more than one may be used to satisfy these requirements (as long as one is stamped and one indicates current enrollment in a New England or New York school)
- b. The visa holder’s I-94 must be endorsed with Duration of Status.

- a. The visa holder must present a DS-2019 (earlier known as the IAP-66) certificate of eligibility.
- b. The visa holder’s I-94 must be endorsed with Duration of Status.

- a. The visa holder must present an I-20 certificate of eligibility.
- The I-20 form(s) must meet the following requirements:

- ii. The I-20 states that the customer is currently enrolled in a Connecticut, Maine, Massachusetts, New Hampshire, New York, Rhode Island, or Vermont school.

Acceptable Immigration Documents
The list of valid immigration documents that may be used in a transaction are as follows. If the document has an expiration date which has passed, the document is not acceptable:
1. Certificate of Naturalization: N-550, N-570, or N-578
2. Certificate of Citizenship: N-560, N-561, or N-645
3. United States Citizen Identification Card: I-179 or I-197
4. Permanent Resident Card (“Green Card”): I-551
5. Temporary Resident Identification Card: I-688
6. Record of Departure (in valid foreign passport): I-94
- a. The I-94 and the valid foreign passport it is within count as only one acceptable document.
7. Processed for I-551 stamp (in valid foreign passport)
8. United States Permanent Resident Re-entry Permit: I-327
9. United States Refugee Travel Document: I-571
10. Employment Authorization Card: I-688B or I-766
11. Documents specific to the customer based upon decisions of the United States Department of State, United States Department of Justice (including Executive Office for Immigration Review, Board of Immigration Appeals and former Immigration and Naturalization Service), or United States Department of Homeland Security (including Bureau of Citizenship and Immigration Services)
All immigration documents presented by the customer must have his/her signature on them.
Residents
In order for a document to prove that a customer is a resident of Massachusetts, it must state the residential address. A Post Office (PO) Box is not an acceptable proof of residence.
The address printed on a document to prove Massachusetts residence must match the residential address that a customer provides during any transaction.
If a customer presents any document that is inconsistent with the concept of his/her being a resident of Massachusetts (such as B1 or B2 or other short-term visa status, or an expired visa status or documents showing an out-of-state residence), he/she will not be considered a resident and an application for a Massachusetts permit/license/ID card will be denied. The customer may be eligible for a Liquor ID card.
A customer who is between the ages of 18 and 21 may use a notarized statement from his/her parent/legal guardian that states the customer currently resides in the parent/legal guardian's house as proof of Massachusetts residence. The following must apply:
- 1. The statement includes the customer's name, license/ID number (if the customer already has a permit/license), date of birth, and address
- 2. The statement includes the parent/legal guardian's name, address, and license/ID number.
The following documents may be used to prove Massachusetts residence:
- 1. Current Massachusetts permit/license/ID card
- 2. Expired Massachusetts license/ID card (expired no more than 12 months)
- 3. Certified or attested copy of a court order of a federal, state, or county court within the United States that contains a raised court seal and includes the customer’s full name, residential address, and date of birth (dated within 12 months of application)
- a. Examples include an adoption document, a name change document, or a gender change document
- 4. Home mortgage, lease, or loan contracts with the customer’s name, residential address, and signature (dated within 12 months of application)
- a. Examples include Retail Installment Sale Agreement, and Motor Vehicle Installment Sale Contract
- 5. Current, valid homeowner’s or renter’s insurance policy with the customer’s residential address that is for the current or immediate prior year
- 6. A bank passbook with the customer’s name and residential address printed or written in a designated place in the passbook
- 7. A bank statement or transaction-related document (no more than 60 days old) on a form generated by the issuing bank and displaying the bank’s name and mailing address, and the customer’s name and residential address, received through mail by the customer
- 8. A checkbook with the customer’s preprinted name and residential address
- 9. ATM/Debit Card statement with name and residential address (no more than 60 days old)
- 10. Canceled personal check (copy acceptable if printed on a sheet with other cancelled checks and issued as part of a monthly statement by the bank) with signature of customer and preprinted residential address on the check and dated not more than 60 days earlier
- 11. Original or certified copy of a United States-issued Marriage Certificate (dated within six months of application)
- 12. A utility bill (gas, electric, wired telephone, wired cable, or heating oil delivery bill) that contains the customer’s name and residential address (no more than 60 days old)
- 13. Current Massachusetts registration card
- 14. A property tax or excise tax bill that is for the current or the immediate prior year and contains the customer’s name and residential address
- 15. Tuition bill or student loan coupons, with residential address, with a due date of not more than 12 months old
- 16. Original school transcript with name and residential address (not valid if more than 12 months old)
- 17. Original letter issued and signed by the principal, headmaster, or official keeper of the records of a Massachusetts school (including a college or university) on the school’s letterhead that states the customer is currently a resident student and includes the name and date of birth (dated within 12 months of application)
- 18. Medicaid correspondence (dated within six months of application)
- 19. Credit Report issued by Experian, Equifax, or TransUnion (dated within 12 months of application)
- 20. Annual Pension or Retirement Statement for the current or immediate prior year
- 21. Annual Social Security Statement for the current or immediate prior year
- 22. First-class mail from any federal or state agency that displays residential address
- 23. W-2 Form from current or previous year that displays residential address
- 24. Cell phone, credit card, doctor, or hospital bill issued within the last 60 days
- 25. Voter registration signed and certified by a city/town official
- 26. Pre-printed pay stub with the employer’s name and address and the customer’s name and residential address (no more than 60 days old)
- 27. Current car loan payment book that displays residential address
- 28. Car insurance policy/bill (no more than 60 days old)
- 29. Current Massachusetts-issued Professional License that displays residential address
- 30. Jury Duty Summons (dated within 12 months of application)
- 31. Census or Census Verification for current year
- 32. Massachusetts Firearms Card, with photograph, signature, residential address, and date of birth (expired no more than 12 months)
- 33. Notarized statement from parent/legal guardian that states customer currently resides in the parent/guardian’s home (only for 18-21 year old customers)
